All workplace accidents that result in an injury must be recorded in the company accident book.

The health and safety executive publish an Accident Book BL 510 that conforms to the Data Protection Act 1998 with regards the control of the personal details of the casualty. All pages are perforated so that individual records of accidents can be removed as soon as they have been completed for confidentiality and safe keeping. Also a check box has been added so that the casualty can give consent for their personal details to be made available to company safety representatives.

Safety representatives have a legal right to request details on workplace accidents. If the casualty refuses to make their personal details available the company will only provide details of the accident excluding any personal information that identifies the casualty.
